Closing May 31, 2019

Dear Patients:
We regret to inform you that effective May 31, 2019, we are closing our practice due to career changes. We will no longer be available to take care of your medical needs after that date.

Dr. MaKieve will be joining Kaiser Permanente in Roseville as a primary care physician and Dr. Najmabadi will be a full-time hospitalist in Auburn.

Please arrange to have another physician take care of you. Dr. Quinn Li and Dr. Jane Teng are both accepting new patients in our same parking lot, or you may use your insurance company's provider directory or check with Sutter Roseville Hospital's physician referral service or the Placer Nevada Medical Society.

Once you have identified a physician or practice, please sign a records release authorization at our office by May 31st to obtain copies of your medical records.

In accordance with California state law, there will be a nominal fee to make copies of your chart to cover copy  and transmittal costs. If you choose to pick up the copy of your record so that you can you may personally take it to your new physician, please do so by May 31, 2019. Starting June 1st, 2019, your records will be in storage,

As of June 1, 2019 all patient records will be held with DATASAFE. They can be reached at 800-275-SAFE.
